Registered Nurse: Assessment, treatment and coordination of healthcare services provided by a caring professional team of nurses who work closely with clients, families, and physicians.

What to EXPECT when you are EXPECTING HOMECARE from Professional Home Nursing

EXPECT... a visit within 24-48 hours of getting home, unless indicated otherwise

EXPECT... to be seen by at least an RN or PT with the possibility of being seen by both and
maybe OT and/ or speech and/ or SW if there is a need

EXPECT...to be educated on your medication, diagnoses, home safety and adaptive equipment or any other needs you may have at home

EXPECT...to understand your Diagnoses, medications, surgical precautions and recommendations on how to modify your home environment if needed

EXPECT...to have assistance in pain management so you can participate in your rehabilitation

EXPECT...to understand and participate in your care or have a significant other be involved to assist with your care when there is a need

What is Skilled Home Care?

It comes after a doctor's visit, a hospital stay or a rehab stay.

Can anyone get Skilled Home Care?

Yes, with a doctor's order for an individual who has a skilled need such as;a new diagnosis,a change in treatment plan for an existing diagnosis, or changes in medications. A hospitalization often will qualify for skilled home care and frequently after a rehab stay, skilled care is needed for a safe transition to home. An individual must be homebound to qualify for skilled home care.

How long will I be receiving Home Care Services?

The duration of the Skilled Home Care Services that you received is individualized to your needs.

What is being Homebound?

Homebound is when it is a taxing effort for client's to leave their home due to their illness/condition. Some examples include a recent surgery, severe shortness of breath upon ambulating less than 20 feet, open wound, leaving your home is medically contraindicated, requiring assistance to leave home due to cognitive impairment or other medical reasons such as limited mobility, generalized weakness/decreased endurance/abnormality of gait, Requiring assistive device, Requiring assistance to rise from a chair, High fall risk, etc..

If I am Homebound does it mean that I cannot go to church or get my hair done?

No, any absence that is infrequent or of relatively short duration such as getting your hair done and going to church is ok.

Why should the doctor's office, hospital or rehab facilities refer to us?

Demonstrated better client outcomes, decreased re-hospitalization or return to rehab facilities within their 30 day post discharge, less opportunity for medication errors, easier transition for clients/caregivers to their home, better continuity of care, meeting the client's needs in the comfort of their home and assessing the need for extra support or devices to increase independence in care.
